Referral Process

We work closely with referring dentists to ensure every patient receives the right care at the right time. Treatment is always carried out within the scope of the referral and patients are returned to their dentist for ongoing care once treatment is complete.
To refer a patient, simply complete the online form below and upload any supporting files, such as radiographs or images. Please include your email address so you receive a copy of the referral form for your records.
